ABSTRACT

OBJECTIVE@#To explore the association between periconceptional supplementation of folic acid or multiple-micronutrients containing folic acid(MMFA) and risk of preterm delivery in women with natural conception, singleton pregnancy and vaginal delivery.@*METHODS@#A retrospective cohort study was performed based on the prenatal health care system and hospital information system of Tongzhou Maternal and Child Health Hospital of Beijing and the women who had their prenatal care in the hospital from January 2015 to December 2018 were included. The information of 16 332 women who conceived naturally, had a singleton pregnancy, and delivered vaginally was collected. Compliance scores were constructed based on the time of initiation and the frequency of taking nutritional supplements. The association between maternal periconceptional micronutrient supplementation, including pure folic acid (FA) pills or MMFA and the rate of preterm delivery was evaluated using Logistic regression models.@*RESULTS@#The preterm delivery rate (gestational week < 37 weeks) of the study population was 3.8%, and the mean (standard deviation) of gestational age was (38.98±1.37) weeks. A total of 6 174 (37.8%) women took FA during the periconceptional period, 8 646 (52.9%) women took MMFA, and 1 512 (9.3%) women did not take any nutritional supplements. The association between periconceptional supplementation of FA or MMFA and risk of preterm delivery in women was not statistically significant [adjusted odds ratio (aOR)=1.01, 95%CI: 0.74-1.37]. The associations with preterm birth were not statistically significant in further analysis by the type of nutritional supplements, time of initiation, and the frequency of supplementation. In addition, the association between the compliance score of taking supplements and the rate of preterm delivery was not statistically significant, either.@*CONCLUSION@#This study did not find an association between the risk of preterm delivery and the use of FA or MMFA during the periconcep-tional period in women with natural conception, singleton pregnancy, and vaginal delivery. In the future, multicenter studies with large-scale prospective cohort or population-based randomized controlled trials are warranted to confirm the association between taking FA or MMFA during the periconceptional period and preterm delivery among women.

ABSTRACT

ObjectiveTo analyze the fingerprint of six pungent herbs based on the molecular connectivity index(MCI)and the matching frequency total statistical moment method, and to study the division and integration of the "imprinting template" of their volatile components, so as to find the common "imprinting template" characteristics of the pungent herbs. MethodThe volatile components of six pungent herbs were extracted by steam distillation, and their fingerprints were established by gas chromatography-mass spectrometry(GC-MS) with a programmed temperature increase(80 ℃ for 5 min, 5 ℃·min-1 to 200 ℃ for 5 min, 2 ℃·min-1 to 230 ℃ for 10 min), a splitting ratio of 20∶1, an electron bombardment ion source(EI) and the detection range of m/z 35-650, and the average MCI and total statistical moment parameters of the fingerprints were calculated. Then the matching frequency method was used to classify, integrate and confirm the chromatographic peaks of the fingerprints of six pungent herbs. ResultThe average zero order, first-order and second-order MCI values of the volatile components of Pogostemonis Herba, Artemisiae Argyi Folium, Atractylodis Rhizoma, Asari Radix et Rhizoma, Magnoliae Flos and Schizonepetae Herba were 9.02, 5.28 and 5.05, respectively. The average values of peak number, total zero-order moment, total first-order moment and total second-order moment were 60, 169×107, 22.49 min and 36.82 min2, respectively. The 20 integrated imprinting templates were obtained by the matching frequency method for the six pungent herbs, among which three were common imprinting templates with the retention times of (25.97±0.21),(26.90±0.20),(31.64±1.24) min, respectively, and the representative components were valencene,β-elemene, caryophyllin, etc. ConclusionMCI combined the matching frequency total statistical moment can divide and integrate the characteristics of imprinting templates of six pungent herbs, and find their common chromatographic imprinting characteristics, which can provide a reference for the determination of effective substances of pungent herbs.

ABSTRACT

Efforts to control inflammation and achieve better tissue repair in the treatment of periodontitis have been ongoing for years. Human β-defensin 3, a broad-spectrum antimicrobial peptide has been proven to have a variety of biological functions in periodontitis; however, relatively few reports have addressed the effects of human periodontal ligament cells (hPDLCs) on osteogenic differentiation. In this study, we evaluated the osteogenic effects of hPDLCs with an adenoviral vector encoding human β-defensin 3 in an inflammatory microenvironment. Then human β-defensin 3 gene-modified rat periodontal ligament cells were transplanted into rats with experimental periodontitis to observe their effects on periodontal bone repair. We found that the human β-defensin 3 gene-modified hPDLCs presented with high levels of osteogenesis-related gene expression and calcium deposition. Furthermore, the p38 MAPK pathway was activated in this process. In vivo, human β-defensin 3 gene-transfected rat PDLCs promoted bone repair in SD rats with periodontitis, and the p38 mitogen-activated protein kinase (MAPK) pathway might also have been involved. These findings demonstrate that human β-defensin 3 accelerates osteogenesis and that human β-defensin 3 gene modification may offer a potential approach to promote bone repair in patients with periodontitis.

ABSTRACT

Angioimmunoblastic T-cell lymphoma (AITL) is one of the most common subtypes of peripheral T-cell lymphomas. It is a fol-licular T-helper-derived neoplasm, and characterized by the presence of some genetic alterations, such as mutations in TET2, DN-MT3A, IDH2, and RHOA. Anthracycline-containing regimens represent the most widely adopted first-line option; however, new biolog-ic agents should be designed and incorporated to improve treatment response. The elucidation of the specific roles of these genetic al-terations in AITL will facilitate the development of molecularly targeted therapies to treat this disease.

ABSTRACT

OBJECTIVE:To screen the agonist active ingredients of peroxisome proliferator-activated receptor-γ (PPAR-γ) in flavonoids from Artemisia ordosica,and provide reference for finding antidiabetic agents in A.ordosica.METHODS:Using known PPAR-γagonist rosiglitazone as positive control,molecular docking technology was conducted for docking one by one for 18 flavonoids and PPAR-7 targets obtained from A.ordosica.It was compared with binding affinities and binding modes of compounds and PPAR-7 targets,and the possible PPAR-γ agonist ingredients in A.ordosica were screened.RESULTS:5 flavonoids showed good docking affinities,in which,compound 3 (5,3',4'-trihydroxy-7-methoxyflavone) showed the highest (-8.3 kcal/mol).Docking mode analysis showed that the phenol oxygen on ring A and ring B of the flavonoids with LBD active site of PPAR-γ formed one (Tyr327) or two hydrogen bonding (Tyr327,Arg288),which played an important role in the binding of flavonoids and PPAR-γ and the stability of PPAR-γ conformation.CONCLUSIONS:Results of virtual screening in molecular docking technology indicate that flavonoids (mostly containing multiple free phenolic hydroxyl groups) in can easily form good docking mode and high affinity with PPAR-γ,showing potential antidiabetic activity.The study can provide reference for further research of chemical ingredients for the treatment of type 2 diabetes.

Objective·To analyse the composition of hospitalization childbirth costs and its influencing factors, which provides theory basis on reasonable control of rapid growth of hospitalization childbirth costs. Methods·With the method of stratified random sampling, 12 secondary and tertiary hospitals were chosen in Shanghai. According to the hospitalization number, the maternal disease cases delivering from Jan. 1, 2016 to Jun. 30, 2016 were randomly selected. The basic information and the cost of hospitalization were collected, and the database was established. Descriptive statistics, z test, non-parametric test and multiple linear stepwise regression analysis were performed by SPSS 20.0. Results·The cost of hospital delivery per capita in Shanghai from Jan. to Jun. 2016 was 7 826.39 yuan, of which the cost of bed, inspection, operation and western medicine accounted for the highest proportion of the expenses (17%, 16%, 15% and 15%, respectively). Maternal hospitalization cost univariate analysis showed that the effect of age, occupation, payment, pregnancy complications, modes of delivery, length of stay, prenatal and postnatal pathological condition on hospitalization cost were statistically significant. A multifactorial study on the cost of hospitalization showed that the delivery mode, length of stay, prenatal and postnatal pathological pathological condition, age and pregnancy complications were the main influencing factors. Conclusion·The hospitalization cost of cesarean section is significantly higher than that of vaginal delivery, the hospital cost structure of these two modes of delivery is different, and the reduction of cesarean section rate is of great significance.It is necessary to optimize the hospital cost structure via controlling the cost of western medicine,operation, and inspection.

ABSTRACT

OBJECTIVE:To observe the clinical efficacy and safety of recombinant human endostatin (rh-endostatin) combined with Lobaplatin for injection and irinotecan injection in the treatment of advanced recurrent small cell lung cancer (SCLC). METHODS:A total of 88 patients with advanced recurrent SCLC in our hospital were divided into control group (41 cases) and observation group (47 cases) according to random number table. Control group was given Irinotecan injection+Lobaplatin for injection. Observation group was additionally given Recombinart human endostatin injection 15 mg added into 0.9%Sodium chloride injection 250 mL,ivgtt,qd,for consecutive 14 d,every 14 d. A treatment course lasted for 28 d,and both groups were treated for 2 courses. The clinical efficacy,the levels of serum carcinoembryonic antigen(CEA),the scores physical state (ECOG) and quality of life (QOL) before,after treatment were observed in the two groups,and the survival and adverse reactions of the two groups were compared. RESULTS:The total response rate of observation group was 59.6%,which was higher than 43.9% of control group,but there was no statistical significance (P>0.05). Before treatment, there was no significant difference in serum CEA levels,ECOG scores or QOL scores,between 2 groups(P>0.05);after treatment,the serum CEA levels of the two groups were significantly decreased,and the observation group was significantly lower than the control group,with statistically significant (P<0.05). In observation group,ECOG scores decreased while QOL scores increased significantly,and significantly better than the control group,with statistical significance(P<0.05). The overall survival(OS)of observation group was 16.8 months,which was significantly higher than 11.5 months of control group,with statistical significance (P<0.05). The incidence of leucopenia in observation group was significantly higher than control group;the incidence of leucopenia and abnormal liver function were significantly lower than control group,with statistical significance(P<0.05). CONCLUSIONS:Rh-endostatin injection combined with lobaplatin and irinotecan can improve serum CEA levels and the quality of living aswell as prolong the survival time.

ABSTRACT

OBJECTIVE:To observe clinical efficacy and safety of Xuebijing injection combined with imipenem and cilasta-tion in the treatment of severe abdominal infection,and its effects on plasma endotoxin and inflammatory factors. METHODS:Dur-ing Apr. 2013-Apr. 2016,100 patients with severe abdominal infection in our hospital were divided into observation group and control group according to random number table,with 50 cases in each group. Both groups were given Imipenem and cilastation sodium for injection 0.5 g added into 0.9% Sodium chloride injection 500 mL,ivgtt(≥40 min),q12 h. Eight hours later,ob-servation group was additionally given Xuebijing injection 100 mL added into 0.9% Sodium chloride injection 500 mL,ivgtt, bid;Both groups were treated for 5-7 d. The levels of plasma endotoxin and inflammatory factors(TNF-α,IL-6,IL-6/IL-10) were compared in 2 groups before after treatment,and clinical efficacies and the occurrence of ADR was recorded. RESULTS:Before treatment,there was no statistical significance in plasma endotoxin or inflammatory factor levels between 2 groups(P>0.05). After treatment,plasma endotoxin and inflammatory factor levels of 2 groups were decreased significantly,and the obser-vation group was significantly lower than the control group,with statistical significance(P<0.05). The excellent and good rate of observation group was 98.00%,which was significantly higher than 78.00%,with statistical significance(P<0.05). No obvi-ous ADR was found in 2 groups. CONCLUSIONS:Xuebijing injection combined with imipenem and cilastation show significant therapeutic efficacy for severe abdominal infection,can effectively control the release of endotoxin and inflammatory factors with good safety.

Objective @# To study the effects of arecolineand Calcium ion (Ca 2+) on the permeability of dysplastic oral epithelia model in vitro.@*Methods@#To establish the dysplastic oral epithelia model in vitro by culturing oral keratinocyte(DOK) and harvesting a DOK cell monolayer. The models were divided into control group, arecoline group and "Ca 2++ arecoline" group. The values of Lucifer Yellow Papp were used to estimate the permeability changes of models after treatedwith arecoline and Ca 2+. @*Results @# In arecoline group, the lucifer yellow Papp values of each subgroup were higher than that of control group (P < 0.05), and the values increased as arecoline concentration elevated and time lasted (P < 0.05). When the "Ca 2++ arecoline" group were pretreated with Ca 2+, the values of subgroup "10 μg/mL" was lower than that of the corresponding arecoline group (P > 0.05). However, the value of subgroup "4 h 10 μg/mL" of "Ca 2++ arecoline" group had no statistical difference with that of control group (P > 0.05), while the other subgroups were increased (P < 0.05); Besides, these values in "Ca 2++ arecoline" group were increased as the arecoline concentration elevated and time lasted too (P < 0.05).@*Conclusion@#Intervention of arecoline contributes to the increase of permeability of DOK cell monolayer model, which maybe an important reason for the cancerization of dysplastic oral epithelia, however Ca 2+might weaken these effects of arecoline in the process.

ABSTRACT

Objective To analyze and compare reflectance confocal microscopy (RCM) features between Paget's disease and eczema on the scrotum.Methods RCM was performed to image scrotal lesions of 5 male patients with suspected Paget's disease and 5 male patients with eczema.Then,the scrotal lesions were resected and subjected to histopathological examination,and a comparison was conducted between the confocal microscopic and pathological findings.Results RCM imaging for the 5 cases of Paget's disease showed disorganized epidermal structure,absence of normal honeycomb structures,single or clustered pagetoid cells in the prickle cell layer.RCM imaging for the 5 cases of eczema showed intercellular edema in the prickle cell layer,reticular degeneration of blisters in all the cases,and pustule formation in some cases.Conclusion RCM features are obviously different between Paget's disease of the scrotum and scrotal eczema,so RCM imaging can facilitate the early diagnosis of Paget's disease.

ABSTRACT

Objective To investigate reflectance confocal microscopy (RCM) characteristics of childhood hypopigmented lichen striatus.Methods RCM was performed to image skin lesions and perilesional normal skin in 11 children with hypopigmented lichen striatus.Then,confocal microscopic findings of the skin lesions were compared with pathological findings.Results Histopathological examination of the skin lesions showed mild intercellular and intracellular edema,thickening of prickle cell layer to different extent,local liquefaction degeneration of basal cells,and perivascular infiltration of plenty of lymphocytes and a few melanophages in the superficial dermis.RCM horizontal images showed multifocal liquefaction degeneration of basal cells,which caused indistinct dermo-epidermal junction,incomplete or unclear dermal papillary rings,infiltration of many highly refractive melanophages and slightly to moderately refractive inflammatory cells in papillary and superficial dermis.Conclusion RCM characteristics can serve as strong evidences for the diagnosis and differential diagnosis of childhood hypopigmented lichen striatus.

ABSTRACT

Objective To investigate the clinical and reflectance confocal microscopy (RCM) characteristics of hypopigmentation induced by local glucocorticoid injection.Methods A retrospective study was carried out on 26 cases of hypopigmentation induced by local glucocorticoid injection.The clinical manifestations,RCM characteristics and treatment outcomes of hypopigmentation induced by local glucocorticoid injection were analyzed in these cases.Results Of the 26 patients,22 were female and 4 were male.Lesions were round or oval,pale white or white patches with obscure boundaries.Different degrees of localized skin atrophy occurred in 15 patients,and subcutaneous venous networks were observed in 8 of the 15 patients.RCM imaging of lesions in the 26 cases showed different degrees of epidermal thinning in the areas of hypopigmentation,shortening or disappearance of trochanterellus,and decreased number or disappearance of chromocytes in the basal layer,which were totally different from RCM characteristics of vitiligo lesions.Conclusion Hypopigmentation induced by local glucocorticoid injection is a kind of depigmentation disease complicated by skin atrophy with definite inducements,and RCM is of great value to its diagnosis.

ABSTRACT

Embryonic stem cells have unlimited proliferative capacity, which may provide a source of tendon stem/progenitor cells for tissue engineering. Experts of International Science and Technology Collaborative Program of Ministry of Science and Technology have developed a protocol consensus on differentiation of human embryonic stem cells into the tendon cells. The consensus recommends a protocol of two-step generation of human embryonic stem cells into tendon cells: the human embryonic stem cells are first differentiated into mesenchymal stem cells on different material surfaces; then with the scaffold-free tissue engineering tendon formed by high-density planting, the mesenchymal stem cells are induced into tendon cells under static or dynamic mechanical stimulation in vivo and in vitro. Tissue engineering tendon established in vitro by the protocol can be used as a model in toxicological analysis and safety evaluation of tendon-relevant small molecule compounds, medical materials and drugs.

ABSTRACT

Objective To investigate the effect of α-keto acid combined with low protein diet on kidney function, calcium-phosphorus metabolism and adenosine deaminase activity in malnourished patients with long term peritoneal dialysis.Methods Forty-six cases malnourished patients with long term peritoneal dialysis were selected from May 2013 to August 2014 in the hospital and divided into two groups.The control group (n=23) were treated by conventional therapy, acceptable daily intake of protein 1.2 g/(kg? d).The experiment group (n=23) were treated by α-keto acid combined with low protein diet, acceptable daily intake of protein 0.8 g/(kg? d).Two groups were treated for three months.Serum calcium, phosphorus, nutritional status assessment, adenosine deaminase activity and renal function were observed and compared after treatment.ResuIts After twelve months ’ treatment, compared with control group, serum creatinine, blood urea nitrogen, serum uric acid of experiment group were lower ( P0.05).ConcIusion α-keto acid combined with low protein diet could effectively increase serum calcium level, reduce serum phosphorus level, ADA activity and WBC count, and improve nutritional status in long-term peritoneal dialysis patients, which has important significance.

ABSTRACT

We conducted in-depth analysis on the use of a popular Chinese social networking and microblogging site, Sina Weibo, to monitor an avian influenza A(H7N9) outbreak in China and to assess the value of social networking sites in the surveillance of disease outbreaks that occur overseas. Two data sets were employed for our analysis: a line listing of confirmed cases obtained from conventional public health information channels and case information from Weibo posts. Our findings showed that the level of activity on Weibo corresponded with the number of new cases reported. In addition, the reporting of new cases on Weibo was significantly faster than those of conventional reporting sites and non-local news media. A qualitative review of the functions of Weibo also revealed that Weibo enabled timely monitoring of other outbreak-relevant information, provided access to additional crowd-sourced epidemiological information and was leveraged by the local government as an interactive platform for risk communication and monitoring public sentiment on the policy response. Our analysis demonstrated the potential for social networking sites to be used by public health agencies to enhance traditional communicable disease surveillance systems for the global surveillance of overseas public health threats. Social networking sites also can be used by governments for calibration of response policies and measures and for risk communication.

ABSTRACT

A 42-year-old woman was admitted to the hospital for an 11-month history of pustular lesions of the mouth and lips as well as a 5-month history of pustules,ulcer,proliferative plaques on the head,neck and trunk.She had suffered from 8 years of ulcerative colitis.Dermatological examination revealed mild erosion of eyelids,swelling of buccal mucosa and oral lips,as well as typical snail track lesions formed by pustules,exudates and mucilage.There was a large pitchy proliferative plaque sized 20 cm × 10 cm with an elevated margin in the neck as well as palm-to persian walnut-sized proliferative plaques in the prothorax,armpit,periumbilical area and left inguinal area.Histopathology revealed epitheliomatous hyperplasia of the epidermis with the formation of eosinophilic microabscesses,and superficial dermal infiltrate with numerous neutrophils and eosinophils.Direct immunofluorescence tests for IgA,IgG,and C3 were negative.A diagnosis of pyodermatitis-pyostomatitis vegetans was made.The patient was given intravenous methylprednisolone 40 mg/day and symptomatic treatment,which leaded to a gradual subsidence of skin lesions.

A mouse model of stem cell mobilization was established by consecutive subcutaneous injection of 100 μg/kg G-CSF for 5 days. The blood from the donor mice was routinely examined during mobilization. Stem cells and MDSCs were analyzed by flow cytometry. The immunosuppressive molecules derived from MDSCs in serum and spleen, including hydrogen dioxide (H2O2) and nitric oxide (NO), and the activity of nitric oxide synthase (NOS) were determined during the mobilization. Apoptosis of T lymphocytes was assessed by using Annexin-V/PI. During stem cell mobilization, the number of lymphocytes and white blood cells in the peripheral blood was increased, and peaked on the 4th day. The number of stem cells in G-CSF-treated mice was significantly greater than that in controls (P<0.01). The expansions of MSDCs were also observed after G-CSF mobilization, with a more notable rate of growth in the peripheral blood than in the spleen. The activity of NOS and the production of NO were increased in the donor mice, and the serum H2O2 levels were approximately 4-fold greater than the controls. Consequently, apoptosis of T lymphocytes was increased and showed a positive correlation with the elevated percentage of MDSCs. It was concluded that G-CSF could provide sufficient peripheral blood stem cells for transplantation. Exogenous administration of G-CSF caused the accumulation of MDSCs in the peripheral blood and the spleen, which could lead to apoptosis of T lymphocytes and may offer a new strategy for the prevention and treatment of graft versus host disease.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect of electroacupuncture (EA) at Fenglong (ST40) on inflammatory factors in macrophages of hyperlipemia (HLP) rats, and to explore the role of EA in treating HLP.</p><p><b>METHODS</b>Totally 50 adult male SD rats were selected. After one-week adaptative feeding, they were randomly divided into 5 groups, i.e., the blank control group (Group A), the model group (Group B), the diet control group (Group C), the EA group (Group D), the diet control + EA group (Group E), 10 in each group. By the end of treatment, all rats were sacrificed to get blood from the carotid artery. The contents of serum total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), and low density lipoprotein cholesterol (LDL-C) were determined. Peritoneal macrophages were isolated. Contents of intercellular adhesion molecule-1 (ICAM-1), monocyte chemoattractant protein 1 (MCP-1), tumor necrosis factor-alpha (TNF-alpha), and interleukin 6 (IL-6) were detected using flow cytometry (FCM) after adding the fluorescein-labeled molecule antibodies of inflammatory factors.</p><p><b>RESULTS</b>Compared with Group A, the serum contents of TC and LDL-C obviously increased and HDL-C significantly decreased in Group B (all P < 0.01). There was no obvious change in the content of TG between Group A and Group B (P > 0.05). Compared with Group B, the contents of TC and LDL-C significantly decreased in Group C, D, and E (P < 0.01). Compared with Group C, serum contents of TC and LDL-C obviously decreased in Group D and E (P < 0.01). Compared with Group B, the content of macrophages significantly increased in Group A (P < 0.01). Compared with Group B, the content of macrophages significantly decreased in Group D and E (P < 0.01), more obviously in Group E (P < 0.01). Compared with Group C, the content of macrophages significantly decreased in Group E (P < 0.05). Compared with Group A, levels of ICAM-1, MCP-1, TNF-alpha, and IL-6 in the peritoneal macrophages significantly increased in Group B (P < 0.01). Compared with Group B, levels of ICAM-1, MCP-1, TNF-alpha, and IL-6 significantly decreased in Group C, D, and E, but more obviously in Group E (P < 0.01). Compared with Group C, levels of ICAM-1, MCP-1, TNF-alpha (P < 0.05, P < 0.01), and IL-6 (P < 0.05) significantly decreased in Group E.</p><p><b>CONCLUSION</b>EA at Fenglong (ST40) could obviously downregulate the serum levels of TC and LDL-C in HLP rats, reduce the contents of macrophages, ICAM-1, MCP-1, TNF-alpha, and IL-6, thus arriving at the therapeutic effect in treating HLP.</p>
